Proof of Vaccine
Have you received your COVID-19 vaccine? Do you know how to get proof of your vaccination?
It is really important for an individual to have access to their vaccination certificate, whether in hard copy or electronically, as it is a entry requirement for most non essential establishments in Victoria.
Staff of these venues are required to check your vaccination status upon entry to keep other patrons and staff safe.
Paper copy of vaccine certificate
A paper copy of your vaccination certificate can be posted out to you by phoning the Australian
Immunisation Register (Monday – Friday, 8am – 5pm) on 1800 653 809
Electronic vaccine certificate
You can also show proof of your vaccination on your smart phone.
First, you must set up a MyGov account. MyGov is a website where you can get your tax information, medical information and your information from other government services.
If you are eligible for Medicare, you need to make sure Medicare is linked to your MyGov account.
If you are not eligible for Medicare, you need an Individual Health Identifier (IHI) to get your proof online. You can do this through MyGov.
If you are a DVA gold card holder and have
a Medicare number, you can also obtain your certificate through MyGov. If you do not have a Medicare number, you can contact My Health
Record online or by calling 1800 723 471

Some venues are requesting photo ID to accompany your vaccination certificate. If you don’t have a driving licence or passport, you can apply for a Proof of Age Card or a Keypass ID. Application forms are available from the post offices, or you
can apply online.
